All candidates must complete and

the early bird registration fee and guaranteed accommodation, you

Accommodation and School costs by

fees and deposit of 50% of their adjusted Accommodation and

All costs are quoted in South
African Rands (approximately R6.75/US$). Participants resident in
South Africa can pay by bank transfer or personal cheque. Foreign
participants can pay by bank draft or bank transfer (in US dollars).
All cheques and bank drafts should be made payable to

| STEP
1 |
| Please complete
the initial registration form
(FORM 1),
to indicate your interest to
attend the 16th Chris Engelbrecht Summer School “2005”.
Participation will be subject to this form being
completed and submitted.
Receipt of this form will be confirmed via e-mail in
which an appropriate
REFERENCE NUMBER will be issued to you, which
must be used in all subsequent correspondence.
Please note that no payments should be made before receipt of
your reference number.

| STEP
2 |
|
The second form
(FORM 2)
can be completed and submitted together with
FORM 1, but this is not
necessary for successful registration and can be submitted at a later
stage. If you are submitting
FORM 2 after having submitted FORM 1 and
have received your reference number, please insert this number in the
appropriate field. However,
if the two forms are submitted together, this field can be left blank.
FORM 2 covers your reservation
of accommodation. It is
stressed that all accommodation at Alpine Heath Resort will be
allocated on a “first paid, first served” basis.
Please note that in order to qualify for “first comer” status,
a payment must accompany the submission of the form.

any changes to the information submitted via FORM 1 and/or
FORM 2 needs to be made,
FORM 3 can be submitted
quoting the
appropriate reference number and detailing the changes.
Upon receipt of FORM 3, an e-mail confirming your changes will be
sent to you. |
